    Job Title: Pharmacist

    Reporting to: Pharmacy Manager

    Contract Type: Permanent and Fixed Term Contracts available 

     

    Job Summary:  To be responsible for the safe, appropriate and cost effective use of medicines. To provide a comprehensive clinical pharmaceutical service.  The fundamental philosophy guiding this position is to promote and preserve the Bon Secours Mission, Values and Ethos.

    Essential Requirements:

    The post holder must be registered with the Pharmaceutical Society of Ireland and remain registered on a continuing basis for the duration of their employment.

    Duties and Responsibilities:

    Subject to the supervision of the Head of Pharmacy Services or his or her nominee, the Basic Grade Pharmacist will undertake duties appropriate to the grade, and participate fully in the provision of a comprehensive pharmaceutical service including:

    • The provision of a comprehensive pharmaceutical service to service users
    • The provision and development of services to the wards, including ward round attendance, Kardex review, patient counselling and medicines reconciliation
    • The purchase, storage and supply of all pharmaceutical items consistent with quality, safety and value for money with reference to legal requirements, transit, security and conditions of chemical and physical stability
    • The maintenance of records for purchasing, quality control, compounding and dispensing to the standards required
    • The provision of advice as required, to medical and nursing staff on the safe, effective and economical use of drugs and medicines, including participation in schemes and educational activities for the dissemination of information in relation to drugs and drug usage.
    • The provision of medicines information and advice services to patients and their families when required
    • Identify, report and act as appropriate on medication safety related risks and incidents and actively participate in risk management
    • Production of annual and other reports as required
    • Ensure health and safety standards are met
    • Participation in the discharge planning of priority patients and liaison with community-based health professionals when deemed necessary
    • Participation in on-going provision of safe systems of generic substitution
    • Participation in policy development under the direction of the Head of Pharmacy Services, to ensure adherence to best practice in medicines use and to ensure legal compliance with new and existing pharmacy legislation.
    • Co-operation with medical and nursing staff and others as appropriate, to facilitate patient safety and patient care in the management of new and existing treatment strategies
    • Participation in multi-disciplinary committees when deemed appropriate by the Head of Pharmacy Services
    • Participation in audit and research as deemed necessary by the Head of Pharmacy Services
    • Assisting the Pharmacy team in the performance of his/her duties as required
    • Create and promote healthy working relationships within the pharmacy department and with other departments throughout the Hospital
    • The undertaking of such other duties as deemed appropriate to the office, as may be assigned to him/her by the Head of Pharmacy Services and other duly authorised officers of the Hospital Board e.g. CEO

     

    Education, Training and Self-Development:

    • To assume responsibility for his/her own professional development and safe work practice and to assist the pharmacy department in maintaining records of continuing professional development as required by the Pharmacy Act 2007
    • Participate in mandatory training programmes
    • Take part in training / supervision of staff / students on placement as appropriate
    • To be aware of current developments and issues in health care by reading current literature and keeping abreast of new developments, attending lectures and courses when possible and as appropriate in consultation with your head of department

     

    Quality, Safety and Risk:

    • Follow agreed policies, procedures and safe practices in the workplace
    • Assist in ensuring the safety of self and others, and the maintenance of safe environments and equipment used in pharmacy
    • Assess and report risk in their assigned area(s) of responsibility including medication error reporting as per pharmacy SOP and organisational policy
    • Support a non-punitive approach to incident reporting relating to medication and patient safety
    • Act on product defect, recall and caution notifications relating to medicines according to pharmacy SOP and as directed to by the supervising pharmacist
    • Take the appropriate timely action to manage any clinical or non-clinical safety incidents or near misses within their assigned area(s)
    • Comply with the Safety, Health and Welfare at Work Act, 2005
    • Take reasonable care to protect her/his own safety, health and welfare and that of any other

    Policy and Procedures:

    The duties and responsibilities of this post will be undertaken in accordance with the Policy and Procedures Guidelines of Bon Secours Hospital, Dublin, which may be amended from time to time.

     

    Person Specification

    • Must be a qualified Pharmacist and registered with the Pharmaceutical Society of Ireland.
    • Ability to attend to detail with accuracy, requiring good organisational and administrative practices
    • Possess excellent problem solving and interpersonal/communication skills.
    • Have an ability to detect errors, follow procedures, and troubleshoot.
    • Ideally, have previous experience at a similar level
